The Lead Care Team Support Specialist is responsible for assisting with scheduling, caregiving support, and client care, with an emphasis on creating extraordinary relationships. Homecare by Callos offers variety of non-medical services that allow seniors to remain in their home and meet the challenges of aging with dignity, care, and compassion. We serve both private pay and Medicaid Waiver/Passport clients in a seven county NE OHIO area. This position can be based in Canton or Youngstown with weekly travel between both locations as needed.
Objective:
- The Care Team Support Specialist is expected to perform a variety of duties in the coordination of care for our clients. The Care Team Support Specialist is responsible for assisting with scheduling clients and Care Professionals, in order to provide the highest quality service. Provide necessary coaching to caregivers, assists with retention and growth efforts by maintaining positive relationships with both clients and caregivers.
Primary Responsibilities:
- Drive Growth, Customer Satisfaction and Retention efforts.
- Assists with Training of other Care Professionals.
- Serves as a liaison between front line team, recruiting, clinical and management.
- Assist with opening new admissions and matching qualified caregivers to new clients.
- Answer incoming calls in a friendly, professional and knowledgeable manner.
- Assist in creating and maintaining client and Care Professional schedules with an emphasis on creating high quality matches, continuity of care, and the development of extraordinary relationships.
- Clock caregivers in and out using EVV. Document properly in client, caregiver charts as well as with client case manager
- Supply any necessary reports to the management team as needed.
- Enter and maintain accurate client and Care Professional records in scheduling software.
- Follow up and communicate Care Professional and client issues to ensure problems are resolved in a timely manner.
- Recognize and capture opportunities to increase services hours to enhance and/or increase quality care.
- Maintain regular attendance at the office to execute job responsibilities
- Demonstrate open and effective communication with Management, office staff colleagues, Care Professionals, clients and family members.
- Adhere to all company policies, procedures, and business ethic codes and ensure that they are communicated and implemented within the team.
- Maintain updated and accurate Care Professional availability and personnel records
- Must be able to provide care for clients, as needed
Secondary Responsibilities:
- Participate as needed in all Care Professional meetings and mandatory trainings and in-services
- Perform any and all other functions and responsibilities deemed necessary
- Must possess and maintain a drivers' license and valid car insurance
- Perform any and all other functions and responsibilities deemed necessary for the successful fulfillment of business needs
Education/Experience Requirements:
- High school graduation or the equivalent
- Two year of Home Health scheduler experience or an equivalent combination of education and work experience may be considered
- Must possess a valid driver's license
- Prior experience in home health industry and/or experience as a caregiver or STNA strongly desired.
