Position Summary
We are seeking a compassionate and resourceful Per Diem Social Worker to support our clients and their families by connecting them with community resources, assisting with care planning, and addressing psychosocial needs. The Social Worker will work collaboratively with agency staff, clients, family members, and community organizations to enhance client well-being and support aging in place.
Responsibilities
- Conduct patient assessments and identify client needs.
- Connect clients and families with community resources, benefits, and support services.
- Assist with new patient care with Structured Family Caregiving.
- Provide emotional support and counseling within the scope of practice.
- Participate in client care planning and case conferences as needed.
- Educate clients and families on available community resources and services.
- Maintain accurate and timely documentation.
- Collaborate with caregivers, healthcare professionals, and community partners to ensure continuity of care.
- Support clients experiencing changes in health, living situations, or caregiver support.
Qualifications
- Bachelor's or Master's Degree in Social Work from an accredited program.
- Current Ohio Social Work license (LSW) required; LISW preferred.
- Minimum one (1) year of social work experience, preferably in home care, senior services, case management, or community-based services.
- Knowledge of community resources and social service programs in the Greater Cleveland area.
- Excellent communication, organizational, and interpersonal skills.
- Ability to work independently and manage a flexible schedule.
- Reliable transportation and current driver's license.
Preferred Qualifications
- Experience working with older adults and individuals with disabilities.
- Knowledge of Medicaid waiver programs and community-based support services.
- Bilingual candidates are encouraged to apply.
